 Thirty five cases of ulcerative colitis have been experienced in the authors’ clinic for the past 10 years. It was very hard to diagnose this disease in its early stage, especially when pathological change was slight and localized. In such cases, conservative treatment including steroid administration has been effective and long-term remission has been obtained in some cases. However, it was found that even in cases of long-term remission, the lesion had already spread to such a degree as to resist all conservative treatments when its recurrence was noticed. Periodical examination and long-term follow up is on that account essential.

 In the cases here presented relapsing-emitting type is most common, but its clinical course is not regular until it shows a fixed patholgical status where surgical treatment is indicated. Though only few in number, the patients of chronic continuous type are more fiable to undergo surgical treatment than those of relapsing-remitting type.

 Steroid treatment has played a main role in the conservative therapy of ulcerative colitis, but method and duration of steroid administration should be determined in each case with strict observation of its effect.

 The fundamental surgical treatment of this disease is total proctocoletomy and ileostomy. To avoid unfavorable disorder due to artificial anus, a new technique is now performed in the authors’ clinic, providing an antiperistaltic segment of the terminal ileum interposed in the ileoproctal an-astomosis.
