スキルスの逆追跡による二，三の知見

A FEW PROBLEMS ON RETROSPECTIVE TRACE OF SCIRRHUS

Ⅰ．はじめに

　スキルスの経過を述べるにあたりまず明らかにしておきたいことは，胃癌の経過を観察することはありえないことである．従ってこのような経過の観察は患者が手術を拒否したまま止むをえず経過を観察せざるをえなかった症例や，切除胃でスキルスと診断された症例について，遡って他医でのX線フィルムを積極的に探し出し，ふり返って検討をしたものおよび見落しの症例の検討である．従ってこのような症例は予め一定の経過期間で観察したものではなく，あくまで逆追跡による，したがって検討にたえる症例数も少なく，未だ結論できるほどの普遍的な内容に乏しいことを断っておきたい．まず私達の経験例を供覧し，二・三の問題点にふれてみたいと思うが，スキルスの経過で問題になるのはまず第1にそのX線診断または内視鏡診断の難かしさであり，第2にその臨床症状または各種検査所見の経過，第3にスキルスの発生に関する問題にまとめてみた．

　ところで，本文で扱かうスキルスとは概括的に結合織性の変化が強く，びまん性発育をきたした癌と解釈して話を進めるが，ほぼ同義的に扱われるものにボールマンⅣ型癌またはLinitis plastica Carzinomatosaがある．主病巣がびまん性・浸潤性発育を示しておれば，一部潰瘍形成がみられたとしてもスキルスとして扱った．

　以前私達はびまん性発育をきたした胃癌例で内視鏡検査を施行したところ，比較的短時日（約1週間）に腹水貯留をきたした数例を経験し，このようなボールマンⅣ型癌例での内視鏡検査では，過剰の空気挿入が胃漿膜側での癌細胞播種をきたす可能性のあることを報告した⁴⁾．その後このような例での内視鏡検査例で同様の経過をとるものは少ないが，少なくとも経過中に比較的急速な進展を示す症例もあり，臨床的に若年者での頻度が高くかつ次第に本型に増加の徴をみることははなはだ重要な位置を示すものと考える（第1表）．

　Investigating five cases on the process of the scirrhus cancer retrospectively, we reviewed few problems concerning with its diagnosis, symptoms and signs.

　The difficulties of the radiological and/or endoscopical diagnosis were realized on the demonstrated cases, owing to the characteristic submucosal infiltration of the cancerous cells in the cases of scirrhuus cancer. Either the direct biopsy or cytological examination were not enough for the diagnosis of the scirrhus cancer. Menetrier's disease or the case of giant hypertrophy of the mucosa must be carefully neglected in the differntial diagnosis of the scirrhus cancer.

　History of patient's complaints must be fully evaluated. Fullness or vague epigastralgia were often elicited from the patient in the early stage of the scirrhus cancer.

　Scirrhus cancer is characterized by poorly balanced, stiff and rigid stomach wall radiologically, and hypertrophied mucosa, bleeding and the erosion with dirty whitish coat on the mucosal surface endoscopically. Nevertheless, these diagnostics were sometimes helpless indiagnosing the scirrhus cancer, following up the patients with lots of vague complaints every year.

　Investigating the process of the scirrhus cancer, relating with the early gastric cancer, type Ⅱc early gastric cancers were revealed to precede the scirrhus cancers in some of them.

　Developement and process of the scirrhus cancer has been unclear. Early detection and early treatment must be aimed for the treatment of this most miserable type of gastric cancer, discussing the above problems.
