  • Abstract

Case: A young woman 22 years of age visited our clinic on July 12, 1969, because of hunger epigastralgia of 6 month's duration. X-ray picture revealed widening and rigidity of the angulus (Fig. 1), and an irregularly shaped niche with nodular formation in and around it, located on the anterior wall of the angulus region (Fig. 4).

By endoscopy a depression of irregular shape was seen at the angle with a deep ulceration in the center and abruption of converging mucosal folds, the whole picture suggesting an early gastric cancer of type Ⅱc+Ⅲ (Fig. 2). Gastric biopy revealed adenocarcinoma. Gastrectomy was porformed on August 19, 1969.

The resected specimen disclosed an irregularly shaped, shallow depression, measuring 4.0×2.9 cm, located on the lesser curvature, 4 cm oral from the pyloric ring. There was a deep ulceration in the center (Fig. 3).

Histological examination revealed infiltration of poorly differentiated adenocarcinoma limited within the mucosa. Microscopically, infiltration of cancer was seen in the area corresponding to the depression noticed in the gross specimen. There was no infiltration of cancer in the base of ulcer. Lymph node metastasis was negative.

Two years later she was delivered of a healthy girl, and now enjoying good health two years and five months after operation.
