肉眼的に奇異な形態を示した炎症性ポリポージスの存在した潰瘍性大腸炎の1例

Ulcerative Colitis Complicated with Peculiar Inflammatory Polyposis in Shape, Report of a Case

　潰瘍性大腸炎は本邦においては漸次増加の傾向にあり，病因，病態の解明はもとより治療面においても多くの努力がなされている^1)～3)．本症は内科的治療に抵抗する重症型や激症型症例も少なくなく，外科的処置を受けるものも本邦では11.8％から20.3％ある²⁾⁴⁾⁵⁾とされている．

　今回，われわれはあらゆる内科的治療にも抵抗を示し，奇異な炎症性ポリープの不規則な形成と頻回かつ多量の粘血便が続いたため外科的切除を行ったところ，海草状あるいは胎盤の分葉化状と形容される形態を示す多発・集簇性炎症性ポリポージスが認められた症例を経験した．いわゆる緩解期にみられるような萎縮・ポリポージス型とは趣きを異にした，極めてまれな続発性病変と考えられたので報告する．

　During the clinical course of a 29-year-old man with ulcerative colitis, a complication with multiple and partly massive inflammatory polyposis like a seaweed or cotytedonen of placenta in appearance, extending from the lienal flexure to the descending colon was observed.

　This inflammatory polyposis was considered to be a rather peculiar lesion different from a mucosal tag or bridge seeming caused by “undermined” ulcer as seen in the so-called abatement stage.

　Treatment of severe or vehement type to perform intensive intravenous regimen at the initial stage and to determine the timing of surgery was considered essential.
