びまん性小腸疾患のX線診断―経口法とゾンデ法二重造影法との対比

Radiographic Diagnosis of Diffuse Small Intestinal Diseases―Comparison between barium meal study and double contrast study

　経口的小腸X線検査は簡易ではあるが，得られる所見は複雑多岐であり，時間の経過に従って刻刻変化するため，微細病変の診断は極めて困難である．斉藤¹⁾は，小腸のX線検査に当たっては胃潰瘍のNischeを発見するごとき，単純でしかも決定的所見を得ることは初めから望み得べくもなく，常に機能的・器質的変化を総合的に観察する必要があると述べている．しかし，白壁・市川ら²⁾は，腸結核について，術後像と合わせて検討し，機能面を除いて恒存する管腔変形から，器質疾患の診断に精力を傾けた．

　ゾンデ法による小腸二重造影法の最大の利点は全小腸が連続して描出され，遮断剤の注射により蠕動の影響を除き粘膜面の微細な凹凸を描出して器質的異常を描出できる点にあると思われる．小腸二重造影法の歴史は古いが³⁾，ゾンデ挿入，造影剤注入などにわずらわしさがあり，また器質的小腸疾患が少ないこともあり，あまり普及しなかった．本邦において，1974年に中村ら⁴⁾，小林ら⁵⁾により経ゾンデ法による小腸二重造影法が短時間で検査を終了でき，しかも全小腸の二重造影像が得られ，器質病変の優れた描出能が得られるとの利点が報告された．ほぼ時を同じくして，クローン病が注目を集める時代が到来し，以後腸結核との鑑別を中心として小腸二重造影法は飛躍的に進歩，普及した⁶⁾⁷⁾．術中内視鏡像，術後小腸造影像との対比により，微細病変の診断も相当に可能となった．しかしながら逆に，ほぼ全小腸にわたるびまん性，連続性の病変を有する疾患（以下，びまん性小腸疾患）のゾンデ法二重造影法によるX線診断についてはまだ十分な検討がなされていない．更に，経口的小腸X線検査においては正常像の検討が詳細になされているが⁸⁾⁹⁾，ゾンデ法小腸X線検査における正常像の検討はほとんどなされていない．

　In normal persons the lumen of the small intestine is stretched by air on double contrast study as compared with that on barium meal study. In view of such pliability of the small intestine, the radiographic characteristics of diffuse small intestinal diseases were studied. The number of the patients studied were seven ; one with acute regional enteritis, one with immuno-proliferative small intestinal disease (IPSID), three with amyloidosis, one with malignant lymphoma of the small intestine, and one with progressive systemic sclerosis (PSS). To know the pliability of the lumen, width of the folds, intravalvular distance and caliber of the lumen were measured in both double contrast study and barium meal study. The results were as follows.

　1) The patients with acute regional enteritis and IPSID showed almost normal pliability in both longitudinal and vertical directions.

　2) The patients with amyloidosis showed variable pliability.

　3) The patient with malignant lymphoma showed impaired pliability in both longitudinal and vertical directions.

　4) The patient with PSS showed no pliability in longitudinal direction and hyperpliability in vertical direction.

　In summary it is considered that comparison of width of the folds, intravalvular distance and caliber of the lumen between double contrast study and barium meal study enables to anticipate, to some extent, the histopathological features of the diseased intestine and that it may be helpful to the differential diagnosis of these diseases.
