 There are still many problems in the treatment for patients with too advanced cancers of the pancreas and biliary tract. A new effective and durable treatment is necessary even if it is palliative, for there is no effective chemotherapy today. Percutaneous transhepatic cholangio drainage (PTCD) has been used with great effect as non-surgical biliary drainage before operation and as permanent biliary drainage in inoperable cases of patients with cancers of the pancreas and biliary tract. Endoscopic retrograde biliary drainage (ERBD), which has been widely used mainly in West Germany, is indeed not easy to perform, but can be done safely and is a kind of biliary drainage which suits the physiological way. Moreover it is an internal biliary drainage by which the drainage tube is completely set in the interior of the patients body. ERBD is, therefore, a non-surgical technique equal to hepato-jejunostomy.

 Authors performed ERBD to ten patients with obstructive jaundice, and succeeded in six patients. But four patients was operated on after biliary drainage and in two, ERBD was performed for the purpose of permanent biliary drainage which is the object of this method.

 ERBD is an ideal method of biliary drainage that is good for not only biliary drainage before operation, but also for aid to inoperable patients enabling them to go back to work.
