大腸の内視鏡診断(3)―過形成性ポリープ

Hyperplastic Polyp of the Colon

　大腸によくみられる上皮性のポリープには，組織学的に2種類ある．腺腫と過形成性（化生性）ポリープである．両者を肉眼的に鑑別することは困難なことが多く，生検組織像によって，はじめて診断できる．将来悪性化する可能性もある腺腫と，発育に限度があり，かつ悪性化のまったく心配のない過形成ポリープとでは，自らその扱いが異なり，両者の正確な鑑別診断は臨床的にきわめて重要である．大腸のファイバースコープのイメージの向上，冷光光源の改良などによって非常に小さな隆起性変化まで詳しく観察することができる上，鮮明な記録写真の撮影，正確な狙撃生検を行なうことが容易になった．過去3年間にファイバースコープで観察した大腸の上皮性のポリープにつき，過形成性ポリープを中心に検討を加えた．なお本邦では過形成ポリープと腺腫の異同があいまいに考えられている傾向があるので，冗長とは思うが，文献的考察を併記した．

　It is generally agreed that there are two apparently distinct and readily separable types colonic polypsadenomatous and hyperplastic.

　We studied about hyperplastic polyps of the colon. All of them (14 cases) were observed fiberscope and were biopsied. Most of them were sessile and less than 5 mm in diameter. The largest one was 10 mm in diameter and its surface color was red. It was difficult to differentiate the polyp from adenoma by endoscopy. But biopsy revealed its hyperplastic nature. It is important to know that hyperplastic polyp of the colon is not important clinically. Some literature were reviewed.
