 A 61-year-old woman, complaining of epigastric pain, with hereditary disposition of gastric cancer was seen in our hospital. x-ray and endoscopic examinations revealed four lesions in the stomach. The lesion No.1 was a giant tumor in the anterior wall of the antrum: lesion No.2 a slight protruding lesion in the greater curvature of th antrum. Lesions No.3 and No.4 were small elevated lesion in the angle.

Pathohistology obtained by biopsy from the lesion No.1 showed undifferentiated lymphosarcoma, the lesion No.3 papillary adenocarcinoma. Gastrectomy was performed. The resected specimen showed that the lesion No.1 was Borrmann 1type, 6×6cm oatcell carcinoma, with brown irregular surface: the lesion No.2, type I early gastric cancer, 1.8×1.4cm, well differentiated adenocarcinoma: the lesion No.3, Borrmann 2 type, 2.4×2.0cm, well differentiated adenocarcinoma. The lesion No.4 was hyperplastic polyp. The patient died of cachexia six months after operation.

 Oat-cell carcinoma of the stomach is very rare and in 1976, only four cases were reviewed in the literature. There have been no other report ssince that time.

 On routine histologic examination, the tumor cells were arranged homogeneously in sheets and incomplete alveolar pattern separated by a delicate fibrous stroma, showing streams of cells and rosettes or ductule-like structures in smaller parts. Most tumor cells were almost uniformly small, round or oval but occasionally spindle-shaped, and had intensely hyperchromatic nuclei and scanty cytoplasm. The nucleoli were not prominent, and mitotic figures were frequent. Moderate permeation of the venules within the gastric wall by the tumor cells was recognized. These histologic features were compatible with those of oat-cell carcinoma of the lung. The silver impregnation by Grimelius' method was significantly positive for most of the neoplasti ccells. Fontana Masson method was negative. PAS and alcian blue stainings were both negative.
