総胆管隔壁形成の1例

Septum Formation of the Common Bile Duct, Report of a Case

　PTC，ERCPなどの直接胆道造影の普及により胆道疾患に対する診断は容易となり，先天性形態異常も術前に数多く発見されるようになってきた．しかし，その中でも隔壁形成は胆嚢に多くみられるのが常で，総胆管についてはほとんど報告がない．

　最近，われわれは諸検査の結果，総胆管隔壁形成を疑い，手術にて摘出確診したので若干の考察を加えて報告する．

　The septum formation of the bile duct is an extremely rare anomaly. This anomaly was found in a 56-year-old female who had the chief complaint of epigastralgia without high fever and jaundice.

　PTC examination disclosed a linear defect transversing the common bile duct.

　ERCP examination also showed a complete obstruction of the common bile duct which was corresponding to linear defect by PTC examination.

　At operation, the common bile duct was removed and abnormal septum was found. Grossly, the septum was 1mm in thickness and the lumen was almost obliterated.

　Histologically, the septum was covered with columnar epithelium and did not reveal significant differences from adjacent duct wall.
