大量出血をみた残胃Billroth II法吻合部隆起性病変の1例―“Gastritis cystica polyposa-Littlerあるいはstomal polypoid hypertrophic gastritis-遠城寺”の症例

　胃腸吻合部には吻合部潰瘍，吻合部癌，吻合口炎，肉芽腫などの病変がみられるが，残胃吻合部の隆起性病変はまれである．196年，Nikolai¹⁾の報告を初めとして約8例が報告され^2)～8)，術後胃，特にBillroth II法の吻合部にみられるが，その組織学的診断名についてはまだはっきりと定められていない．われわれは，38歳の男性が12年前十二指腸潰瘍のためBillroth II法による胃切除術を受けたが，吻合部より大量出血が生じたため，残胃再切除を行った．この出血は，吻合部の隆起性病変からの出血と考えられ，この病変は組織学的にはLittler²⁾のgastritis cystica polyposaに一致したので，文献的考察を加えて報告する．

　A 38-year-old man who had undergone Billrolh II gastrectomy for duodenal ulcer 12 years ago was admitted because of massive melena. Emergency gastroscpy revealed a polyopoid lesion at the site of gastrojejunostomy, and regastrostomy was carried out. Surgical specimen displayed a polypoid tumor along the anastomotic site measuring 1.8cm across, 6cm in length and 2cm in height and also a scar of jejunal ulcer just distal to the site of gastrojejunostomy. Microscopic examination revealed the so called“gastritis cystica polyposa” at the gastrojejunal stoma.
