内視鏡的に経過を観察しえた虚血性腸炎の1例

Ischemic Colitis Observed by Endoscopy

　患　者：69歳，男．

　主　訴：粘血便．

　家族歴，既往歴：特記すべきことはない．

　現病歴：8年前より高血圧症（170/90mmHg）と冠動脈硬化症として某医で治療を受けていた．ある日突然，胸部圧迫感と心悸亢進が出現し，一過性の血圧低下（80/50mmHg）と共に，軽度の意識障害を来した．翌日全身状態は回復したが，3日目から下腹部の不快感と頻回の粘血便をみたため，精査の目的で当科へ紹介され入院した．

　The patient was a 69 year-old male who was suffering from hypertension and arteriosclerosis and was treated for several years. One day, he went into a shock state due to hypotension. He recovered the next day, and three days later, he discharged tarry bloody stools and was admitted to the department mentioned above. A barium enema and endoscopy examination showed that there were large shallow ulceration in the sigmoid colon. Biopsy and angiography were carried out, and together with the symptoms and clinical signs, ischemic colitis was diagnosed. Treatment was conservative, and scars were seen after three months.
