 A 48-year-old man was admitted to our hospital because of the tender mass and pain of the right-lower abdomen.

 Barium enema and barium-filled pictures showed several nodules and small ulcerative lesions in the terminal ileum and the deformity and niche-like shadow of the caecum.

 The resected specimen showed the adhesion between the caecum and the terminal ileum and a tiny fish bone was found in the adhered region.

 It also showed several nodules and erosions accompanied with mucosal atrophy in the terminal ileum.

 Histologically, several fistulas were found between the peri-cecal abscess and the mucosa of the terminal ileum.
