 This report describes a case with cancer of the rectum occurring during the longstanding course of chronic ulcerative colitis. A 62-year-old woman with chronic ulcerative colitis who had been treated with medications for the past 35 years complained of bloody stool and visited Niigata University Hospital where rectal cancer was pointed out. Following proctosigmoidectomy, the relationship between carcinoma and chronic ulcerative colitis was in particular examined on gross and histologic basis. The tumor was well-circumscribed, almost circumferential with central ulceration and situated at 5 cm proximal to the anal ring measuring 5×3×2 cm in dimensions. It was classified as B2 in the Dukes-Astler's classification.

 Diffuse pseudopolypoid lesions without definite ulceration or active inflammation were present toward the proximal portion of the specmien resected; the changes were most prominent in the sigmoid colon but minimal around the tumor in the rectum and grossly free along the line.

 Histologic examination showed well-differentiated papillotubular adenocarcinoma of the rectum and regenerative type of colitis with multiple foci of healed erosion, most prominent in the sigmoid colon and definite in the rectum surrounding the tumor as well.

 Although a few foci of highly atypical epithelium were present in the midst of regenerative epithelial changes in the sigmoid, definite transition from such changes to the tumor could not be recognized.
